1、甘肃中医学院附属医院(以下简称招标人) 已经从美国获得一笔贷款,用于采购一批医疗设备。
中国技术进出口总公司作为招标代理代表招标人为甘肃中医学院附属医院购置医疗设备项目邀请合格投标人进行密封投标。可以由经授权的代理人代表制造商进行投标,条件是必须随投标文件一同提交一份原始制造商出具的授权函,授权指定的代理人单独代表原始制造商进行投标。所有投标人都必须提交证明材料证明他们有相应的资格、经验和能够按时成功地履行合同的能力。
2、采购货物名称及数量:医疗设备(详见第II卷技术部分)。
所有投标人应对整个包设备投标报价,如果投标人仅对包中的单项或几项设备投标,将导致废标。
3、合格投标人的最低资格和经验要求如下,如任何投标人不满足以下最低资格和经验要求,其投标将被拒绝。
①投标人国别应是美国,在美国注册并且开展业务3年以上,具备从美国出口的经验。近两年(2013,2014)年平均营业额不低于2500万美元。投标人应提供相关证明文件,包括但不限于一套注册登记的营业范围、营业执照和所有相关的注册登记文件以及经审计的近3年(2012,2013,2014)来的资产负债表、损益表和现金流量表复印件等(需同时提供中文翻译)。
②在开标日前,投标人应为此项目获得美国进出口银行出具的意向函,作为打包商资质证明。
③在过去三年(2012,2013,2014)中,投标人在中国完成至少2个利用美国贷款采购医疗设备项目,单个合同金额最低不少于500万美元。投标人应提供一套相关书面证明文件复印件,如合同复印件、合同生效通知和验收证书等。
④投标人应为此项目在开标日前3个月内由投标人开户银行开具的银行资信证明正副本各一份。银行资信证明中需注明为此项目提供的信用额度。信用额度应不低于投标总价。如果在银行资信证明中没有显示为此项目提供的信用额度和/或银行资信证明由2个以上银行提供,投标将被拒绝。
只接受单一投标人投标,不接受联合体投标。
只接受在中国技术进出口总公司正式登记并购买招标文件的投标人递交的投标文件。
4、 有兴趣的潜在投标人可从2015年7月23日起至2015年7月30日北京时间每个工作日(周末和公共假日除外)的上午9时至11时下午13时30分至16时30分(北京时间)在中国北京市丰台区西三环中路90号通用技术大厦2107室(100055)购买招标文件。本招标文件每套售价为10000元人民币或1700美元,售后不退。我公司可接受邮购申请,但在任何情况下不承担招标文件因邮递迟交货或灭失的责任。邮购须另加600元人民币(国内)或200美元(国外)。
在购买招标文件时需提交以下资料(复印件需加盖单位公章或被授权人签字)。资料扫描件请发电邮至:zoujunfeng@cntic.genertec.com.cn。此资料仅用于购买招标文件,不作为开标、评标的依据。如任何潜在投标人没有提供下述资料,将不能购买招标文件。
①法人授权委托书正本、经办人本人身份证原件及复印件。
②一套注册登记的营业范围、营业执照和所有相关的注册登记文件复印件。
③一套经审计的近3年(2012,2013,2014)来的资产负债表、损益表和现金流量表复印件(需同时提供中文翻译)。
④一套相关书面证明文件复印件,如合同复印件、合同生效通知和验收证书等,证明在过去三年(2012,2013,2014)中,投标人在中国完成至少2个利用美国贷款采购医疗设备项目,单个合同金额最低不少于500万美元。
5、所有投标书应于 2015年8月13日上午9时30分 (北京时间)之前递交到招标机构指定地点。在此时间后递交的标书将不被接受。
6、兹定于 2015年8月13日上午9时30分 (北京时间)公开开标,开标地点在北京丰台区西三环中路90号通用技术大厦会议室(邮编100055)。届时请参加投标的投标人出席开标仪式。

For
Medical Equipment Project for Affiliated Hospital of Gansu TCM College under U.S. Ex-Im Bank Guaranteed Loan
1. Affiliated Hospital of Gansu TCM College (hereinafter referred to as the Tenderee) has obtained a loan from the United States of America towards the cost for procuring medical equipment.
China National Technical Import & Export Corporation as the tendering agent and on behalf of the Tenderee hereby invites sealed bids from eligible bidders for the goods and related services for medical equipment project of Affiliated Hospital of Gansu TCM College. Bids shall be submitted by an authorized agent for and on behalf of the primary manufacturer provided the bid is accompanied by a letter of authority from the manufacturer authorizing the designated agent to bid solely for and on behalf of the manufacturer. Documentary evidence will be required of all bidders that they have qualifications, experience and capacity to be able to successfully complete the contract on time for the Goods offered.
2. Goods to be purchased and Quantity: medical equipment (details see Volume II Technical Part)
Bidders should quote the whole package of the above goods. If Bidders only quote one item or several items of the whole package of the goods, the bids will be rejected.
3. The minimum qualification and experience requirements for the eligible bidders are as follows. If any bidder can not meet the following minimum qualification and experience requirements, the bid will be rejected.
①The Eligible Nationality of the Bidders shall be the United States of America. The Bidder shall be registered in U.S.A, developed business for more than three years and has experience of export from U.S.A. The average annual turnover of the Bidder should be more than USD 25 Million in the past two years (2013,2014). The Bidder shall provide the relevant documentary evidence, including but not limited to one copy of the registered scope of the business, business license and all related registration documents as well as the audited balance sheet, profit & loss statement and the cash flow statement for the last three (3) years (2012,2013,2014),etc.(The Chinese translation shall be also provided.)
②Before bid opening time, the Bidder shall have the Letter of Interest issued by U.S. Ex-Im Bank for the project as documentary evidence of eligible bidder.
③The Bidder shall have successfully concluded at least two medical equipment procurement projects financed by American Loan in China with a minimum contract amount of not less than USD 5 Million per contract in the past three years (2012,2013,2014). One copy of documentary evidence for the above projects shall be provided by bidders such as copy of the Contract, the effectiveness notification of the Contract and the Acceptance Certificate, etc shall be attached.
④The Bidder shall provide a letter of Bank Reference issued within three (3) months prior to the deadline of submission of the bid in one original and in one copy, in which the line of credit for the project should be stated by the bank. The requirement of line of credit should be not less than the bid price. If the credit amount for the project is not indicated in the bank reference letter and/or the bank reference letter is issued by more than one bank, the bid will be rejected.
The bidder must be one company. The joint bidding will be rejected.
Only Bids received from Bidders duly registered with CNTIC when buying the Tendering documents will be accepted.
4. The prospective Bidders who are interested in this project are kindly requested to contact China National Technical Import & Export Corporation for obtaining tendering documents between 9:00 a.m. to 11:00 a.m. and 1:30 p.m. to 4:30 p.m. on working days from July 23, 2015 to July 30, 2015 Beijing time (Weekend and public holidays excepted) in Room 2107, Genertec Plaza, No.90 Xisanhuan Zhonglu, Fengtai District, Beijing, China, 100055, upon non-refundable payment of RMB10000 or USD1700. We can accept the request from the applicant for dispatch by registered mail, but under no circumstances will it be held responsible for late delivery or loss of the documents so mailed. For mail order, an extra RMB600.00 (domestic) or USD200.00 (abroad) shall be added.
The prospective Bidders should provide the following documents before obtaining the tendering documents (The duplicate documents should be with official seal or signed by the Attorney). The scanning copy of the following documents should be sent to E-mail address: zoujunfeng@cntic.genertec.com.cn by E-mail. The documents shall be only used in purchasing the tendering documents and shall not be a part of the bid document for bid opening and bid evaluation. If any prospective Bidder does not submit the following documents, the Bidder can not purchase the tendering document.
①One original of the Power of Attorney, the original and one duplicate copy of Identification card of the responsible person.
②One copy of the registered scope of the business, business license and all related registration documents.
③One copy of the audited balance sheet, profit & loss statement and the cash flow statement for the last three (3) years (2012,2013,2014). (The Chinese translation shall be also provided.)
④One copy of documentary evidence, such as copy of the Contract, the effectiveness notification of the Contract and the Acceptance Certificate, etc. certifying that the Bidder shall have successfully concluded at least two medical equipment procurement projects financed by American Loan in China with a minimum contract amount of not less than USD 5 Million per contract in the past three years (2012,2013,2014).
5. The deadline for submission of bids is before or at 9:30 a.m. (Beijing time) on August 13, 2015. Bids received after that date and time would not be accepted.
6. Bids will be opened in public at 9:30 a.m. (Beijing time) on August 13, 2015 at the place of conference room, Genertec Plaza, No. 90 Xisanhuan Zhonglu, Fengtai District, Beijing, China, (100055). Bidders are kindly requested to be present.
